A case note is more than paperwork

A useful case note helps the next authorised reader understand the support provided, the person’s communication and choices, and what may need to happen next.

That takes more than a template. It takes judgement about what is relevant, how to describe what happened and when information needs to be followed by action.

Case Notes with Purpose helps you develop that judgement through practical explanations, realistic support-work examples and opportunities to reflect on your approach.

The focus is not on complicated language or recording every detail.

It is on creating a clear, respectful and useful record of the support someone received.

What will you learn?

Across the 10-part course, you will build your confidence in five areas.

Know what to record - and where it belongs

Understand the purpose of a case note, who may legitimately rely on it and how it differs from a handover, incident report, medication record or other documentation.

Describe what happened clearly

Separate observations from assumptions. Attribute information accurately, describe the support provided and avoid labels or unsupported conclusions.

Keep the person visible

Record relevant communication, choices, participation and outcomes respectfully. Include the information needed to understand the support without collecting unnecessary private details.

Recognise when action is also needed

Understand when writing a case note is only one part of the response, and when action, reporting or escalation must also occur.

Build a consistent approach

Use practical tools to review your notes for clarity, accuracy, relevance and usefulness to the next authorised reader.

Created specifically for support work

Case Notes with Purpose focuses on the documentation decisions support workers make in everyday practice.

Developed by SWAA, the course draws on Australian legislation, standards and practice guidance, with references documented across all 10 modules.

The learning brings those expectations into practical support-work situations, with an emphasis on dignity, rights, safety, continuity and accountability.

The aim is to strengthen professional judgement - not simply provide another template to copy.

Short lessons, practical learning

The course includes 10 video modules, with video lessons generally running for 5–15 minutes each. Allow additional time for reflection activities and knowledge checks.

Every video includes captions and a written transcript.

The course also includes:

  • Realistic support-work examples and worked rewrites.

  • Reflection activities to connect the learning with practice.

  • Short knowledge checks.

  • Downloadable quick-reference guides and practice tools.

Move through the available lessons at your own pace and return to them throughout your access period.

Who is this course for?

Case Notes with Purpose is for support workers across disability, aged care and community services - whether you are new to documentation, reviewing an established approach or strengthening your independent practice.

People preparing to enter support work are also welcome.

No previous formal documentation training is required.

For employers and teams, the course offers a shared learning focus on clear, respectful and useful documentation. Contact SWAA to discuss access for your workforce.

Receive a certificate of completion

Complete all 10 modules and achieve 80% or higher on the five-question final quiz to receive your certificate of completion.

That means answering at least four of the five questions correctly.

You can revisit the lessons and retake the final quiz as many times as needed during your access period.

Your certificate recognises completion of the course and can be kept as a record of your ongoing professional development.

This is non-accredited professional development, not a nationally recognised qualification or a formal assessment of workplace competency.
